Map showing the catchment area for the practice

The practice provides medical services to patients living in Wellingborough and the villages of Little Irchester, Hardwick, Great Harrowden, Little Harrowden, Orlingbury, Wilby and Mears Ashby.  The above diagram shows the area that we cover.  If you are unsure whether you are in our practice area please call us on 01933 423424. Please note, we do not accept registrations if you are already registered at a practice in Wellingborough.

Please download the registration form and questionnaire that applies to you below:

Once you have completed the forms, you will need to attend the surgery, in person, with two forms of ID including a photo ID and one with your current address on that’s dated within the last 6 months e.g. utility bill, council tax but not a mobile phone bill. A birth certificate or passport will be accepted for children under the age of 16.

Please note that all patients have a named, accountable GP but can choose to see any of the Doctors in the practice although we do encourage patients to see their "usual" Doctor as this ensures continuity of care. However, you will be offered an appointment with the most suitable Clinician depending on your problem.
