Before filling out a registration form, check you are in our catchment area.

The practice provides medical services to patients living in Wellingborough and the villages of Little Irchester, Hardwick, Great Harrowden, Little Harrowden, Orlingbury, Wilby and Mears Ashby.

Registering with the surgery

You can use our online registration service to register as a new patient. If you are registering a child under the age of 16, you will need to provide a copy of the birth certificate.

Please be aware that both the GMS1 registration form and the adult or child registration form needs to be completed. Registration forms will need to be brought to the surgery in person with 2 forms of ID (photographic and proof of address).

Please note that all patients have a named, accountable GP but can choose to see any of the doctors in the practice although we do encourage patients to see their usual doctor as this ensures continuity of care. However, you will be offered an appointment with the most suitable clinician depending on your problem.

When you register, it’s helpful to have your NHS number. You can use the NHS website to find your NHS number.
